Fields of application of the active substance: simvastatin

Simvastatin is used to reduce elevated cholesterol values in the blood, if a low fat and low-cholesterol diet, a weight loss and physical training of improvement are not enough. Simvastatin causes a drop of “bad” LDL cholesterol and a relative increase of vascular protective HDL cholesterol. The administered dose of active ingredient is typically between five milligrams to 40 milligrams once a day, depending on, how much the LDL cholesterol should be reduced.

Is the concentration of cholesterol in the blood is lowered by simvastatin, it can accumulate any more in the form of so-called plaques in the blood vessels. In this way, the active ingredient protects against changes and hardening of the blood vessel walls (arterial circulatory disorders), as well as heart attack or stroke.

After vascular replacement heart (bypass surgery), the new blood vessels, especially in high-risk patients can clog quickly. Such patients include those who to diabetes (diabetes mellitus) or are suffering from a disease of the coronary vessels, or already had a heart attack. Simvastatin can help to keep open the bypass. To prevent an active ingredient quantity of 20 to 40 milligrams of simvastatin, is captured each in the evening.

A hereditary conditional increase of well-controlled-Gehalts (Hyperlipidemia type IIa, IIb and IV to common medical Division) simvastatin is used in addition to a diet, if the diet alone causes no sufficient reduction of blood lipids. The active ingredient is taken up this once in the evening in the amount of 40 milligrams. An active ingredient quantity of 80 milligrams of simvastatin (taking 20 mg morning, 20 mg at noon and 40 mg at night) is only used for very severe increase in the blood lipids (severe hypercholesterolemia) or very high risk for a closure of the coronary vessels. With an effective entrance is to be expected two weeks after starting treatment. In coronary artery disease, the treatment should be maintained to at least over a year.

Following applications of simvastatin, more detailed information are available:

  • Lipid metabolism
  • Stroke
  • Heart attack
  • Diabetes mellitus type 2
  • Diabetes mellitus type 1
  • Heart and cardiovascular problems

Effect of simvastatin

Simvastatin is one of the active group of statins. It is used for the reduction of elevated cholesterol values in blood and prevent vascular deposits at the same time. This reduce the risk for stroke and heart attack.

Simvastatin effect primarily in the liver, where the blood lipids and among them also the cholesterol, are produced. Cholesterol is transported in various forms in the blood, bound to proteins of low density (as LDL cholesterol) or proteins of high-density (HDL cholesterol). From the form of LDL can easily dissolve the cholesterol and deposited on the walls of the blood vessels. This form is therefore especially dangerous. The HDL form, however, can resume even mounted off cholesterol and thus reduce deposits. It is in this way protecting on the vessels.

Simvastatin effect on the forms of transport of cholesterol differently. It lowers blood levels of “bad” LDL cholesterol by more than 45 per cent and can provide the “good” because tank protective HDL cholesterol by 15 percent increase. This improves the ratio of lipids to each other. This can be done both increased Cholesterinspiegeln and normal Cholesterinspiegeln.

Simvastatin reached its maximum concentration in the blood quickly, after about one or two hours. A proven change of the cholesterol values occurs however until approximately two weeks after the start of therapy and reaches its maximum after four to six weeks.

For the most part on the liver and the digestive tract and to a smaller extent of the kidney excretion of simvastatin via urine.

By clinical studies, it has been confirmed that simvastatin lowers the risk of deadly events such as a heart attack or a stroke in heavily.

Simvastatin may be used at the same time with:

  • Antibiotics (erythromycin, clarithromycin, telithromycin)
  • Nefazodone (for depression)
  • Fungal agents (ketoconazole, itraconazole)
  • the funds to build muscle Danazol
  • Medications for HIV (HIV-1 protease inhibitors: nelfinavir, saquinavir, ritonavir, indinavir, amprenavir).

Reason for this is that these funds and simvastatin effect on the same mechanism of metabolism in the liver. While taking simvastatin would significantly increases the concentration in the blood and significantly increased the risk of complications in skeletal muscle. Also the metabolism in the liver the antihypertensive agents of Verapamil and diltiazem, the Amiodarone employed against cardiac arrhythmias, the vitamin niacin (at doses of 1 G per day) affect the blood fat-lowering Fibrate (Bezafibrat, Gemfibrozil, Fenofibrate) and the drug Cyclosporine, omitted the body’s defenses. To reduce the risk of diseases of skeletal muscle, the following shall apply:

  • Danazol, Cyclosporine, niacin and Fibrate should be taken at the same time with more than ten milligrams of simvastatin per day.
  • Verapamil and especially Amiodarone should be applied with more than 20 mg simvastatin per day. Sometimes otherwise muscle wasting, subsequently even to renal failure within a so called rhabdomyolysis, sometimes fatal. Alarm are colafarbener urine and muscle pain
  • Diltiazem should be applied with more than 40 milligrams of simvastatin per day.
  • Grapefruit and grapefruit preparations should be waived during treatment with simvastatin completely. Grapefruit and grapefruit preparations increase the risk for complications on the skeletal muscle because they also hinder the simvastatin removal.

The blood clotting time is extended when taking simvastatin with orally taken Blutgerinnungshemmern (Cumarinen). Therefore a regular medical examination of blood clotting should be done at the beginning of therapy. Patients taking much to much alcohol to (alcohol abuse), simvastatin may apply only in particular medical care because it can cause more liver damage.
