Effect of amitriptyline
Amitriptyline belongs to the tricyclic antidepressants drug group and has their mode of action.
All of the brain’s nerve reactions are mediated by Messenger substances such as serotonin and norepinephrine. These messengers are stored in small vesicles in the nerve endings and are awarded for excitation of nerve. So they get to other nerves and bind there on special binding sites from (receptors). As a result, the affected nerves forward the stimulus signal. After they have fulfilled their task so, be the messengers of special transport systems again in their store recorded or degraded by enzymes.
Amitriptyline has a two-fold effect. Once the active ingredient in the brain occupied different binding sites for messengers and changed so that the influence of these messengers to the brain metabolism. Secondly, amitriptyline transport substances, again carrying back the messengers serotonin and noradrenalin after successful signal transmission in their memory blocks. As a result remain serotonin and noradrenalin longer on the drugs and increases their effectiveness. Amitriptyline solves this way anxiety and voltage States and repeals depressive upsets. The soothing effect of amitriptyline occurs usually very quickly while the mood enhancing adjuvants effect uses only after about two weeks of regular use.
In chronic pain, the pain signals in the brain processing is often disturbed. The neurotransmitter serotonin plays a major role in setting up a filter for incoming pain signals. Through the influence of serotonin, amitriptyline may replace the constant intake of painkillers. The drug must be taken then regularly for pain relief for a long time.

Interactions between amitriptyline are especially with active substances which have a also consciousness dampening effect, such as tranquilizers and sedatives. These resources, but also alcohol, there may be a mutual effect enhancement by amitriptyline.
A mutual reinforcement of efficiency is achieved with active substances which have anticholinergic effects such as amitriptyline. These include phenothiazines (a group of psychiatric drugs), Antiparkinsonmittel such as Biperiden, antihistamines (used as a sleep aid and against allergies), or atropine (included for example in eye drops).
Cimetidine (against stomach ulcers) and methylphenidate (for example in hyperactive children used) increase the effects and side effects of the active substance amitriptyline.
Some gefäßverengenden resources (Catecholamines), even if they occur only as an addition to the local anaesthetic means, be strengthened by amitriptyline in their effect on the nervous system.
Special medical caution is required if prior or simultaneously antidepressants of other groups of active ingredients have been taken with amitriptyline or be. This applies in particular for the active ingredients of fluoxetine and fluvoxamine, but also for other tricyclic antidepressants. Here it comes with concurrent administration of amitriptyline to the mutual strengthening of the effect and the increase in the incidence of side-effects. To increase dose of amitriptyline and/or the other antidepressants should be adjusted by your doctor.
Antidepressants from the Group of the MAO-inhibitor allowed due to severe side effects not together with amitriptyline. The revenue must be completed at least 14 days before taking amitriptyline MAO-inhibitors. Even with a reverse switch from amitriptyline to MAO-inhibitors, this minimum distance of 14 days applies.
Antipsychotic drugs, which also affect the mind, strengthen amitriptyline in its effect. This also applies to the concomitant treatment with AEDs such as benzodiazepines or barbiturates. Here, seizures of central nervous system can occur more and more.
Amitriptyline may reduce the action of some drugs against high blood pressure such as, for example, Guanethidine, Reserpine, Betanidin, clonidine, or revoke. The same applies to the active ingredient Alpha-Methyldopa.
Special medical care be taken when simultaneous administration of thyroid hormones as undesirable cardiac adverse effects can be amplified.
Active substances against cardiac arrhythmia of type of the Amiodarone or quinidine are reinforced in their effect by amitriptyline. Active substances that speed up the reduction of amitriptyline in the liver, such as barbiturates, the Epilepsiemittel of carbamazepine and phenytoin, such as nicotine or hormonal contraceptives reduce the effect of amitriptyline. At the same time, carbamazepine and phenytoin are reinforced in their effect by amitriptyline.
With concurrent administration of blood clotting agents of coumarin type such as, for example, Phenprocoumon, there may be changes in blood clotting. The coagulation values must be verified therefore regularly by doctors.
